Turns out, Blaidd is just waiting in Siofra River in his spot there, I think his dialogue when talking to his spirit at Ranni's even leads to that. I double checked the Evergaol location before going to Siofra so you might want to do that too incase he is there.

Assuming you talked to him in the ruins, you have to enter the evergail first and the summon sign will be there close to where you spawn. You can summon him when you've entered the Evergaol, he's got a yellow sign on the ground. It's not coming out for me.

it was the black knives that killed him. notice when you approach blaidd at ranni's tower, there's a dead assassin laying in front of him. i think the black knives had found ranni and stumbled upon seluvis first. when the assassin went to get ranni he was met by blaidd and cut down right before he lost his mind.
